The online process provides the option to submit the form, either electronically by providing an electronic signature or printing a PDF copy, to be signed and submitted by mail or fax. If your mailing address is a foreign address or FPO/APO then the pdf (fill in) form should be used.
Please Note: If title is under a company or trust, only requests made by
the owner on title or an authorized agent will be accepted for processing. Unsigned
and/or incomplete submissions will NOT be processed. Please ensure that all
required information is provided. The form must be signed in the name of the individual
submitting the address change request and NOT the name of the company or
trust on title.
